Process
- Patient or their caregiver performs their INR test from home or office.
- Patients call the PPM Service™ program HIPAA compliant toll-free phone number and follow the automated instructions to log their current
INR result into the system.
- The PPM Service™ system confirms patient identity using their unique ID security number for authentication. When prompted, the
patient or caregiver enters their INR result.
- The PPM Service™ system accesses the patient's records to identify the prescribed range determined by their physician.
- The result is automatically entered into the patient's profile, the result is immediately relayed to the managing physician via fax,
email and/or web-interface, depending on the provider's preference.
- The physician is notified of their patient's Out-of-Range result for immediate intervention.
- If the patient's INR test result is Out-of-Range, the patient is routed through the office's pre-determined process
including connecting the patient into the office in real-time.
- All patients are instructed of their next scheduled INR test date through the PPM Service™ system, according to the practice's recommendation.
Patient compliance is enhanced through our auto-reminder, HIPAA compliant messaging service to home testing patients.
The service recognizes individual patient testing schedules and eliminates your need to follow up with home testing patients.
